For both of my kid's teachers, we picked up some gift cards for Godiva. Of course I couldn't just give them the generic little card that came with the Gift card. ;)
The base of this is just a simple matchbook style card. I stapled the bottom about 1/4" up from the fold. This is where the top will slide down into to hold the card closed.The fun part was just embellishing the front to go with the 'Sweet' theme.
